Product Registration Cards let the consumer provide their contact information to the manufacturer. This lets consumers be quickly notified in the event of a safety alert or a recall. 

Products That Require Product Registration Cards

Product Registration Cards are required for all durable infant or toddler products per 16 CFR 1130, including the following products and others:

• Crib mattresses
• Changing pads
• Strollers
• Play yards

Product Registration Card Uses

Product registration cards have a single use: to improve the effectiveness of recalls. Please have the following information ready before submitting your order:

• Manufacturers name and address
• Manufacturers website and phone number
• Model name
• Model Number
• Manufacture date (mm/yyyy)
• First class mail permit number
